Provide Documentation

In order to access services and supports through Disability Services you must provide current documentation to verify your disability. You may wish to use our Disability Services Documentation Form, which clearly outlines what is required. You do not have to send this with your application but it is recommended that you do so, particularly if you require extensive supports that may take time to put in place (i.e. interpreting services). If you have any questions about your documentation contact the CDS nearest you.
