Acclaim Award Application

Applicant/Nominator Check List:

• Completed Award Application.
• Provide two letters of references - one professional reference and one volunteer. The nominator can provide one of these reference letters. (max. one page per letter)

Application, letters of reference and relevant media sources must be received by the CNA Alumni & Advancement Office no later than May 17, 2024

Note: The CNA Alumni Acclaim Awards Selection Committee will independently review and rank each application according to the award criteria and scoring rubrics. The Selection Committee will make the recommendations and reach final approval by a majority vote. All applications will be confidential.

I hereby make the following declaration:

1. I have read and understood the award criteria described in this application, and I believe that I meet all the requirements.
2. I have answered all questions, which are applicable, and the answers given are true.
3. Permission is hereby granted for the Alumni Acclaim Awards Selection Committee to obtain any further information required from appropriate individuals or agencies, if necessary.
4. I further acknowledge that my personal information (e.g., photo, video, name, program and home community) may be shared and used by CNA for promotional purposes.
